Graphic Design Request Form

Before we begin a graphic design project, we require that you submit this Graphic Design Request Form. We will review your request and contact you if we have questions. Our goal is to help you reach your audience while maintaining University brand standards. We may suggest alternative ways of doing things based on our experience.


After submitting this form, you will be taken to a confirmation screen (and also an email) that includes a link to submit your text/artwork.

Some important notes:
  • Do not complete this design request form until you have all of the text, graphics, etc. needed for your project.
  • Only events listed in the Lewis Master Calendar can be promoted on digital signage. To get an event into the Lewis’ master calendar, visit Make sure the master calendar includes contact information, registration information and other important event details.
  • Timelines vary by the complexity of the project. Many take anywhere from 2-3 weeks for completion and will be worked on in the order they are received. Please plan accordingly, and be as thorough as possible when filling out this request. All copy should be proofed for correct spelling, grammar, and punctuation prior to submitting this request.

College-specific and/or Marketing & Communication monitors. This artwork [1920 x 1080 pixels] will appear 1-up on the monitors.
