Admission Process

STEP 1

Contact your Centegra Health System HR Representative to indicate your interest in the Centegra Health System partnership with Lewis University. You will need to complete the “Centegra Health System Employee Discount Form.” This form can be downloaded by clicking on the link below. It can also be obtained from your Human Resources Department.

STEP 2

Complete and submit the appropriate Lewis University application form along with your Centegra Health System Employee Discount Form. Application forms can be found by clicking on the link below:

STEP 3

Request official transcripts from all colleges attended.
