Mental Health Screening

The screening tools on this site are for educational purposes and are not intended to be used by individuals under 17 years of age. If you are under 17 and are interested in learning more about mental health services, we encourage you to talk to your parents or guardians about seeing a physician or qualified mental health professional for a complete evaluation.   By using the screening tools on this site, you are indicating that you are at least 18 years of age.

Please note that screenings do not provide a diagnosis and are not substitutes for a complete evaluation, but rather an indication that someone may be suffering from a mental health disorder. Screenings allow students to learn more about his/her mental health status and the opportunity to connect with the services provided by Lewis University.
