Pre-Arrival Information

Admission Packet Documents

Upon acceptance to Lewis University, we will email and mail you an admission package, including:

Acceptance Letter
You will need this acceptance letter to obtain your visa, and will also need to present this upon arrival in the USA along with your I-20 forms.

I-20 Immigration Forms (or F-1 Transfer Clearance Form)
Your I-20 forms are required to obtain your visa. If you have already attended a school located in the United States, the F-1 Transfer Clearance Form must be filled out by your former school's International Advisor and delivered to Lewis before we can create your I-20.

Pre-Arrival Steps

SEVIS Fee for Obtaining Your Student Visa
Before making an embassy appointment to interview for a visa, you must pay the SEVIS fee of $200 USD. Once you receive your I-20 form and letter of admission, and have paid your SEVIS fee, make an appointment with your local Embassy to apply for your student visa (F-1 status). Note: If you currently possess a valid F-1 visa and plan to transfer from a school located in the United States, you may disregard this step.

Student Housing Application (if you plan to live on campus)
Please mail the Student Housing Application form, along with a check for $100 made out to "Lewis University".

Letter of Intent to Enroll (for undergraduate degree students only)
Please mail the Letter of Intent to Enroll, along with a check for $100 made out to "Lewis University".

Health Immunization Records
All Lewis University students need to submit written proof of updated immunization and tuberculosis (TB) screening/testing. Students are required to have Illinois state-mandated immunizations for measles, mumps, rubella (MMR) and for diphtheria-tetanus-pertussis (DT/DTP/DTaP/Td/Tdap). These Health Immunization Records must be completed and signed by a health care provider before beginning classes.

Confirmation of Your Arrival
Once you have received your student visa or provided us with the F-1 Transfer Clearance Form, you may proceed to make travel arrangements to the United States. Please confirm the date and time of your arrival with the Office of International Student Services: Michael Fekete (feketemi@lewisu.edu) or Tatiana de Lima Machado (delimata@lewisu.edu). We can make arrangements to pick you up from Chicago O'Hare International or Midway Airport when you provide us with your flight details, including:

  • date of arrival

  • name of last airport and city where you will be before coming to Chicago
  • name of arrival airport in Chicago (Midway or O'Hare)

If you would prefer to make your own transportation arrangements, please inform us you will not require transportation assistance.
