Asset Clarification FormThe Asset Clarification Statement confirms asset amounts for net worth of investments, net worth of current businesses and/or investment farms and current personal assets. This form should only be completed if specifically requested by Financial Aid Services.

Income ReductionAn Income Reduction is a petition for review of the FAFSA due to a loss of income resulting from Loss of Employment, Loss of Earnings due to a Disability or Natural Disaster, Loss of Untaxed Income, Separation/ Divorce, or Death which has resulted in an income loss in excess of $10,000.

Independent VerificationThe Independent Verification form is required when a student is selected for verification. By signing your FAFSA you authorized Financial Aid Services to verify the accuracy of your completed form before awarding Federal Aid. Verification compares information from the FAFSA with signed copies of the verification form, the student and spouse’s (if applicable) Federal tax forms, W-2s and other financial documents.
