Asset Clarification Statement
The
Asset Clarification Statement
confirms asset amounts for net worth of investments, net worth of current businesses and/or investment farms and current personal assets. This form should only be completed if specifically requested by Financial Aid Services.
Authorization
The
Authorization Form
is required for all students who are receiving financial aid. This form includes authorization statements regarding FERPA, Title IV Funds, and Title IV Credit Balance refunds.
Dependency Appeal
A
Dependency Appeal
is a petition for independent status based on extreme and sufficiently documented circumstances. Willingly living independently of parents or parents’ refusal to help/contribute to student’s education is NOT considered grounds for an appeal.
Income Explanation - Dependent
The
Dependent Income Explanation
form will be requested by Financial Aid Services when information entered on the FAFSA is insufficient and/ or requires explanation. It should be completed using the PARENT(s)’ information.
Income Reduction
An
Income Reduction
is a petition for review of the FAFSA due to a loss of income resulting from Loss of Employment, Loss of Earnings due to a Disability or Natural Disaster, Loss of Untaxed Income, Separation/ Divorce, or Death which has resulted in an income loss in excess of $10,000.
Medical & Dental Expenses
The
Medical & Dental Expenses
form is a petition for review of the FAFSA due to unusual medical or dental expenses in excess of 11% of the 2008 household Adjusted Gross Income.
Parent in College
The
Parent in College
form is for those dependent students who have a parent currently attending an accredited college or university, are considered to be at least half-time status and are actively seeking a degree.
Revision
The
Revision Form
should be completed if you are requesting a revision to your current Financial Aid Award.
Elementary/Secondary Tuition
The
Elementary/Secondary Tuition
form is for those students or parents (dependent student) who paid private elementary or secondary tuition from January 1 to December 31 in the prior year.
Verification - Dependent
The
Dependent Verification
form is required when a student is selected for verification. By signing your FAFSA you authorized Financial Aid Services to verify the accuracy of your completed form before awarding Federal Aid. Verification compares information from the FAFSA with signed copies of the verification form, student and parent(s)’ Federal tax forms, W-2s and other financial documents.
Verification of Homelessness
The
Verification of Homelessness
form is required based on the answers the student submitted for questions 58-60 on their FAFSA. This form should only be completed if it has been requested by Financial Aid Services.
