Verification

I have been selected for verification – what does this mean?

A student can be chosen for verification either randomly by the Department of Education, or in the case of conflicting information, by Lewis University. In order to continue with processing a financial aid file all documentation must be received by Financial Aid Services in a timely matter. If you have been selected for verification documentation we will require:

  • Verification form completed and signed
  • Student’s Federal Tax form signed
  • Student’s W-2’s
  • Parent(s) Federal Tax form signed (dependent student)
  • Parent(s) W-2’s
  • Student’s spouse’s Federal Tax form signed (independent student)
  • Spouse’s W-2’s

Please Note - Failure to complete and return all required documentation will result in delays in awarding financial aid.
