1) Recommendation Request

All students applying to programs in the Health Professions are required to use the standardized form (attached) to make their request(s). Please realize that an official copy any standardized test scores (i.e., MCAT, GRE, OAT, DAT, etc.) needs to accompany this request form. Requests need to be submitted to the Health Professions Advisor, Dr. Roberts, or to Dr. Vander Vliet if applying to the programs at Northwestern Memorial Hospital, no less than 30 days prior to the deadline.
